New Patient Registration
To access our services you must first be registered with the practice. Please register with the practice closest to where you live.
You can register with the practice if you live within our practice area and are a permanent UK resident.
Our receptionist will ask you to complete a registration form and health questionnaire. Two forms of identification will be required. One being photographic (driver's licence or passport), and the other a utility bill or bank statement dated within the last 3-months. If you have your NHS medical card please bring this along wich will help us register you quickly and efficiently.
New patient registration forms are available from reception or can be downloaded from the above "Registering Online" tab. Once you have filled out the registration form online, print the form and bring it to the surgery so that we can get you registered or send it to us online.
Change of Details
If you move house or change your telephone number please let reception know as soon as possible. If you move whilst undergoing hospital treatment you should also advise that hospital of your new address. You will need to register with another surgery if you move outside our area. Our receptionists will be able to advise you. If you want to change your name, we will need proof of the 'new name' your are changing to i.e passport, birth certificate or a name deed for us to photocopy for your records to ensure this is done correctly and changed within the NHS service.
Please either post your change of information to us or inform our reception team at the time of your next visit.
